چکیده

The operationalisation of this definition consists of two elements: the absence or low intensity of eight core symptoms of schizophrenia (severity criterion) and the maintenance of this state for a minimum of 6 months (time criterion). The consensus definition is aimed at facilitating research on the course of illness, at improving the comparability of studies and at helping clinicians and patients focus on positively formulated treatment outcomes. The definition has been used in several studies of schizophrenia, exploring cognitive ability and antipsychotic medication, and is validated by several studies using functional outcome, symptom severity, need for care and/or quality of life as outcome measures. However, these outcome measures are mostly clinician-rated and standardised, whereas real-life validation using self-report measures of actual functioning (e.g. time spent with others and in goal-directed activities) and real-life experiences is lacking. We therefore aimed to validate the remission criterion using a momentary assessment strategy investigating symptom level and functioning in patients with schizophrenia spectrum disorders. We investigated positive symptoms, negative symptoms and functional outcome in the flow of daily life in patients meeting the severity criterion of the symptomatic remission definition, patients not meeting that criterion and a healthy comparison group, the last allowing a comparison with a ‘normal’ level of functioning. Remission in schizophrenia: validity, frequency, predictors, and patients' perspective 5 years later

In March 2005, the Remission in Schizophrenia Working Group (RSWG) proposed a consensus definition of symptomatic remission in schizophrenia and developed specific operational criteria for its assessment. They pointed out, however, that the validity and the relationship to other outcome dimensions required further examination. This article reviews studies on the validity, frequency, and predict...

An early improvement threshold to predict response and remission in first-episode schizophrenia.

BACKGROUND Early improvement with treatment is thought to be important in patients with first-episode schizophrenia, yet a valid definition is still outstanding. AIMS To develop a valid definition of early improvement and test its predictive validity regarding response and remission. METHOD We examined 188 in-patients with first-episode schizophrenia. Early improvement was defined as improv...

Remission in schizophrenia: proposed criteria and rationale for consensus.

New advances in the understanding of schizophrenia etiology, course, and treatment have increased interest on the part of patients, families, advocates, and professionals in the development of consensus-defined standards for clinical status and improvement, including illness remission and recovery. As demonstrated in the area of mood disorders, such standards provide greater clarity around trea...

Functional magnetic resonance imaging correlates of memory encoding in relation to achieving remission in first-episode schizophrenia.

BACKGROUND Previous studies in schizophrenia have shown a strong relationship between memory deficits and a poor clinical outcome. However, no previous study has identified the functional neural correlates of memory encoding in relation to remission. AIMS To determine whether functional magnetic resonance imaging (fMRI) activation patterns differed between individuals that later achieved remi...

Testing definitions of symptom remission in first-episode psychosis for prediction of functional outcome at 2 years.

BACKGROUND To determine the clinical relevance of different definitions of symptom remission for prediction of functional outcome in first-episode psychosis (FEP). METHODS One hundred forty-one individuals receiving treatment for an FEP at a specialized early intervention service had positive and negative symptoms and functional status rated every month over the first 2 years of treatment usi...
